I registered our baby girl as a member of ANA Mileage Club Family Account Service. We can aggregate each mileage when we exchange the mileage for flight tickets. Since my daughter only has her mileage club card (not ANA credit card), I needed to send (1) completed application form and (2) a photocopy of residence certificate ("jyuminhyou") to them.
