I made a flight reservation from Tokyo to Hawaii(Oahu). Since I and my wife will go with our baby girl, I have to book our seats via telephone (ANA Mileage Club Service Center) instead of the website:

I've confirmed my reservation by telephone today because today is the deadline to fix the schedule.
(1) Airline tickets (round trip) for me and my wife: 40,000 x 2 = 80,000 miles
(2) Additional tax and fee for me and my wife: 39,780 JPY (including fuel surcharge 25,200 JPY)
(3) Airline ticket (10% of the regular fare) and so on for our baby girl : around 15,000 JPY
(4) A place for a cradle: reserved
